your first visit you will meet with our registration
staff prior to seeing a doctor. Please bring your
current insurance card(s) and insurance information.
It
is very important that you bring with you all information
related to a previous diagnosis and treatment of
your injury or condition. This may include X-rays,
test results, chart notes, current medications or
operative reports.

HealthCare
Midwest Osteoporosis Center
The HealthCare Midwest Osteoporosis Center offers
bone density testing and patient education of Osteoporosis.
A bone density test (sometimes referred to as a DEXA
scan) is the most useful procedure for the diagnosis
of Osteoporosis and the evaluation of fracture risk.
HealthCare Midwest has International Society of Clinical
Densitometry (ISCD) certified personnel.
Additional Services
Laboratory
The HealthCare Midwest medical laboratory is fully accredited
by CLIA and COLA and is under guidance of Medical Director,
Dr. David Doan. Upon orders from a physician, laboratory
personnel conduct sophisticated tests using automated equipment
to help the physician determine the nature of a patient's
illness and the proper course of treatment. The laboratory
services include clinical chemistry, clinical hematology,
urinalysis, coagulation, immunology, and microbiology.

may be filled at the pharmacy of your choice.
New
prescriptions and renewals must be requested
during our office hours so that your physician
can review your medical records.
Generally,
prescription renewals will be completed within
24 hours; however, the patient must be presently
under the care of a HealthCare Midwest Internal
Medicine physician. When calling for prescriptions,
please have the pharmacy phone number available
and notify us of any medication allergies or sensitivities.

business hours. Patients are seen by appointment
only.

If you are unable to keep your appointment, please call our
office as soon as possible during regular office hours. We
will reschedule your appointment and release your original

physicians handle many emergencies and it may occasionally
be necessary for us to change your appointment.
Under these circumstances, we will try to contact
you promptly to notify you of changes or delays. Please
be sure to supply our office with a daytime phone
number where you can be reached. We will
do our best to accommodate you by scheduling a
prompt and convenient reappointment.

Our
office participates with a variety of insurance
carriers. Since these arrangements change from
time to time, we will review our participation
status at the time of your registration appointment.
If you have a question about pre-authorization,
please contact your insurance carrier prior to
your appointment.
You
must bring your insurance card(s) to each appointment to
help us provide the most efficient and trouble-free
processing of your claim. If you are not insured,
we request payment at the time of service.
If
your insurance changes, please be sure to notify
us as soon as possible. Having accurate information
enables us to contact you promptly and submit claims
accurately.
HealthCare
Midwest has many physicians and service locations.
For your convenience you will receive one statement
for all HealthCare Midwest services. Any payment
made after the time of service will be applied
to your oldest balance.

If
you have an emergency that requires immediate attention,
call 911, or proceed directly to the hospital emergency
room.
If
you call our office for emergency problems after
regular business hours or on holidays, you will
be given an opportunity to speak to our message
center. The operator will ask you a few questions
and will contact the physician on call.
