Services:

Gastroenterology involves diagnosing and treating problems in the organs of the gastrointestinal tract including the esophagus, stomach, liver, gallbladder, pancreas, small intestine, colon and rectum.

Prescriptions

Prescriptions may be filled at the pharmacy of your choice. 

New prescriptions and renewals must be requested during our regular office hours so that your physician can review your medical records.  

Generally, prescription renewals will be completed within 24 hours;
however, the patient must be presently under the care of Dr. Falk. When calling for prescriptions, please have the pharmacy phone number available and notify us of any medication
allergies or sensitivities.

 

Office Hours:

On your first visit you will meet with our registration staff prior to seeing the doctor. Please bring your current insurance card(s) and insurance information. It is very important that you bring with you all information related to a previous diagnosis and treatment of your condition. This may include X-rays, test results, chart notes, current medications or operative reports.

Appointments may be scheduled by calling our office during regular business hours. Patients are seen by appointment only.

  • Regular Office Hours:
    Monday through Friday
    8:00 am to 12:00 pm
    1:00 pm to 5:00 pm


  • Changes or cancellations:
    If you are unable to keep your appointment, please call our office as soon as possible during regular office hours. We will reschedule your appointment and release your original appointment time to another patient.

    Please be sure to supply our office with a daytime phone number where you can be reached. We will do our best to accommodate you by scheduling a prompt and convenient reappointment.

  • Driving Directions:

    From the North
    Take US-131 South toward Kalamazoo. Merge onto I-94 East
    (Exit #36A). Follow I-94 to the Sprinkle Road Exit (Exit #80). Take a left turn onto Sprinkle Road (North). Continue straight until you come to the light at Gull Road. Turn left onto Gull Road. Turn left into the parking lot immediately past Frosty Boy. Large building to the back of the parking lot. The office is the first suite to the right inside the front door.

    From the South
    Take US-131 North toward Kalamazoo. Merge onto I-94 East
    (Exit #36A). Follow I-94 to the Sprinkle Road Exit (Exit #80). Take a left turn onto Sprinkle Road (North). Continue straight until you come to the light at Gull Road. Turn left onto Gull Road. Turn left into the parking lot immediately past Frosty Boy. Large building to the back of the parking lot. The office is the first suite to the right inside the front door.

    From the East
    Take I-94 West toward Kalamazoo. Follow I-94 to the Sprinkle Road Exit (Exit #80). Take a left on Cork and merge into the right lane. Turn right onto Sprinkle Road (North). Continue straight until you come to the light at Gull Road. Turn left onto Gull Road. Turn left into the parking lot immediately past Frosty Boy. Large building to the back of the parking lot. The office is the first suite to the right inside the front door.

    From the West
    Take I-94 East toward Kalamazoo. Follow I-94 to the Sprinkle Road Exit (Exit #80). Take a left turn onto Sprinkle Road (North). Continue straight until you come to the light at Gull Road. Turn left onto Gull Road. Turn left into the parking lot immediately past Frosty Boy. Large building to the back of the parking lot. The office is the first suite to the right inside the front door.

Insurance:

Our office participates with a variety of insurance carriers. Since these arrangements change from time to time, we will review our participation status at the time of your registration appointment. If you have a question about pre-authorization, please contact your insurance carrier prior to your appointment.

You must bring your insurance card(s) to each appointment to help us provide the most efficient and trouble-free processing of your claim. We request payment at the time of service if you are not insured or if we do not participate with your insurance.

If your insurance changes please be sure to notify us as soon as possible.  Having accurate information enables us to submit claims accurately.

HCMW has many physicians and service locations.  For your          convenience you will receive one statement for all HCMW services. Any payment made after the time of service will be applied to your oldest balance.

Emergencies:

If you have an emergency that requires immediate attention, call 911, or proceed directly to the hospital emergency room.

If you call our office for emergency problems after regular business hours or on holidays, our phone system will give you instructions as to how to contact the physician on call.
